Descripción anatomoquirúrgica de la vascularización e inervación del plano osteofascial de la región temporoparietal, en cadáveres de fetos y adultos, de la cátedra II de anatomía humana normal de la Facultad de Medicina. UNNE
Resumen
La humanidad, desde el siglo XX y lo que va del siglo XXI se caracterizó por la presentación de numerosos y nuevos desafíos para las ciencias de la salud. Uno de ellos se relaciona con la necesidad de brindar a los pacientes que sufren defectos craneofaciales por diferentes patologías congénitas, traumáticas, tumorales, u otras, la solución de las pérdidas óseas faciales, a través de cirugías reparadoras/ reconstructivas seguras, estéticas y funcionales, por la visibilidad del sector involucrado. Esto implica la aplicación y el desarrollo de conocimientos anatómicos para la optimización de técnicas quirúrgicas ya existentes o el desarrollo de nuevos procedimientos que permitan mejores resultados. En cabeza la “Región Temporoparietal”-terminología anatomoquirúrgica-se utiliza habitualmente como zona dadora de tejidos para la reparación de defectos corporales, en especial, de la cara, el cráneo y también de otras regiones del cuerpo humano.
El objetivo general de esta tesis es: describir los aspectos anatomoquirúrgicos de la vascularización e inervación del plano osteofascial de la región temporoparietal en cadáveres de fetos y adultos, que tengan relevancia anatomoquirúrgicos para la confección de un colgajo fascial u osteofascial.
Los objetivos específicos son: a) Localizar los elementos vásculo-nerviosos de la región temporoparietal determinando su constancia y variaciones b) Especificar las ramas perforantes vasculares profundas y el sitio de máxima concentración de estas, c) Caracterizar la vascularización de los planos profundos de la región temporoparietal y ángulos de rotación del eje vascular.
El estudio se encuadra dentro de los diseños metodológicos no experimentales, observacionales, descriptivos y transversales con enfoque cuantitativo. Partimos de la hipótesis de que la vascularización del plano osteofascial de la región temporoparietal es constante y suficiente, y que tienen una importancia fundamental para la confección de un colgajo aplicable en cirugías reconstructivas de la cabeza u otro sector del cuerpo humano.
Se utilizaron 50 cadáveres formolizados y frescos de fetos y adultos. Pertenecieron al sexo masculino 32 (64 %) y al sexo femenino 18 (36 %). Los cadáveres fetales fueron 35 (70 %), fetos masculinos: n = 20 y fetos femeninos: n = 15; y los adultos 15 (30 %), adultos masculinos: n = 12 y adultos femeninos: n = 3. Se utilizaron cadáveres fetales formolizados en todos los casos, y cadáveres adultos formolizados y frescos .
Se realizaron 65 disecciones de la región temporofacial, 35 disecciones del plano osteofascial en fetos (n=35), seleccionando un solo hemilado; y 30 disecciones (15 derechas y 15 izquierdas) del plano osteofascial temporoparietal en adultos (n=15).
La edad promedio de los especímenes fetales fue de 28 semanas de gestación y de los adultos fue de 62 años.
El eje vascular temporal superficial integrado por la arteria temporal superficial (ATS) y la vena temporal superficial (VTS) estuvieron presentes en todos los especímenes fetales y adultos. La ATS es una de las ramas terminales de la arteria carótida externa (ACE) y se hace superficial a la altura de una línea trazada del borde superior del conducto auditivo externo a la porción media del borde superior de la órbita, por delante del trago. Asciende verticalmente a la región temporoparietal, en la parte profunda del tejido celular subcutáneo y en íntima relación con la fascia temporal superficial. En adultos y en promedio, a 23 mm por encima del borde craneal de la raíz longitudinal del proceso cigomático del hueso temporal se divide en sus (2) dos ramas terminales en la gran mayoría de los especímenes.: a. Temporal Frontal (ATS F) y a. Temporal Parietal (ATS P). Estas dos ramas se dirigen hacia la región epicraneal, e irrigan a la galea, al tejido areolar laxo subyacente y al plano óseo compuesto por el periostio y el hueso parietal. La irrigación de estas últimas estructuras se realiza a través de numerosas perforantes, que, en su gran mayoría, emergen de la parte distal de ambas arterias terminales (media 51,6 mm). Además, la ATS proporciona tres ramas colaterales, una de ellas es la a. Temporal Media (ATS M), que perfora la fascia y el músculo temporal llegando al periostio en la fosa temporal, estableciendo redes anastomóticas con las arterias perforantes de la (ATS F y P). Es inconstante. El ángulo de rotación del eje vascular temporal superficial utilizando la ATS F y la vena Temporal Frontal (VTF) puede ser hasta 120°, permitiendo cubrir el hemilado homolateral del cráneo y la cara, sin generar plegamientos vasculares que lo podrían hacer inviable.
La inervación del n. auriculotemporal (NAT) en la región temporoparietal coincidió con las descripciones clásicas, siendo su relación más importante, primero dorsalmente con la VTS y la ATS, y luego de la división de estas últimas, con la ATS P y la vena Temporal Parietal (VTP) siendo sustantivo para la sugerencia de las bases anatómicas de su aplicación quirúrgica.
Como conclusión, se puede afirmar que la originalidad de la propuesta radica en que a partir de la ubicación de los territorios de mayor vascularización del plano osteofascial de la región temporoparietal, se identificó la mejor área utilizable. Esta se localiza en la parte laterosuperior del cráneo, a partir de un punto inicial 2 cm. por fuera de la sutura sagital y 2 cm. por detrás de la sutura coronal. La extensión máxima fue, de ventral a caudal de ≤ 13 cm., y de medial a lateral de ≤ 5cm., obteniéndose en total una superficie máxima promedio de 65 cm2. Esto permite confeccionar un nuevo colgajo osteofascial, cuyo eje vascular está dado por los vasos temporales superficiales, utilizando los numerosos rambos arteriales perforantes distales de una rama terminal (ATS F o ATS P) y el buen drenaje venoso (VTS o VTP) y sus afluentes. Este colgajo está constituido por: fascia temporoparietal superficial, galéa epicraneal, tejido areolar laxo subyacente y el plano óseo: periostio, cortical externa y diploe del hueso parietal y mejora los resultados anatomo-funcionales y estéticos tanto en la zona dadora como receptora al aplicarlos en cirugías reconstructivas de cara y cráneo. Otra aplicación del conocimiento obtenido sería optimizar los colgajos de la región ya existentes que son numerosos. Since the 20th century and so far in the 21st century, humanity, has been characterized by the presentation of numerous new challenges for health sciences. One of them is related to the need to provide patients who suffer craniofacial defects due to different congenital pathologies, traumatic, tumorous, or other, the solution of facial bone loss, through surgeries characterized by being reconstructive, aesthetic and functional reparative/reconstructive, due to the visibility of the sector involved. This involves the application and development of anatomical knowledge for the optimization of existing surgical techniques procedures or the development of new procedures that allow better results. On the head, the “Temporoparietal Region” – anatomosurgical terminology – is commonly used as a tissue donor area for the repair of body defects, especially of the face, the skull and also other regions of the human body.
The general objective of this thesis is: Describe the anatomosurgical aspects of vascularization and innervation of the osteofascial plane of the temporoparietal region in fetal and adult cadavers, which have anatomical and surgical relevance for the preparation of a fascial or osteofascial flap.
The specific objectives are: a) Locate the vascular-nervous elements of the temporoparietal region, determining its constancy and variations b) Specify the deep perforating branches vascular vessels and the site of maximum concentration of these, c) Characterize the vascularization of the deep planes of the temporoparietal region and the rotation angles of the vascular axis.
The study is framed within methodological designs non-experimental, observational, descriptive and transversal with a quantitative approach. We start from the hypothesis that the vascularization of the osteofascial plane of the temporoparietal region is constant and sufficient, and that it is of fundamental importance for the preparation of a flap applicable in reconstructive surgeries of the head or other sectors of the human body.
Fifty formalized and fresh cadavers of fetuses and adults were used. They were male sex 32 (64%) and female sex 18 (36%). The fetal corpses were 35 (70%), fetuses male: n = 20 and female fetuses: n = 15; and adults 15 (30%), male adults: n = 12 and female adults: n = 3. Formalized fetal cadavers were used in all cases, and adult cadavers formalized and fresh.
In fetuses 65 dissections of the temporofacial region, 35 dissections of the osteofascial plane (n=35), selecting a single sides; and 30 dissections (15 right and 15 left) of the plane temporoparietal osteofascial in adults (n=15) were performed.
The average age of the fetal specimens was 28 weeks of gestation and that of the adults was 62 years old.
The superficial temporal vascular axis composed of the superficial temporal artery (STA) and the vein superficial temporal (VTS) were present in all fetal and adult specimens. The STA is one of the terminal branches of the external carotid artery (ECA) and becomes superficial at the level of a line drawn from the upper edge of the external auditory canal to the middle portion of the upper Edge of the orbit, in front of the tragus. It ascends vertically to the temporoparietal region, deep in the subcutaneous cellular tissue and in close relationship with the superficial temporal fascia, above the cranial edge of the longitudinal root of the zygomatic process of the temporal bone and in average in adults it divides into its (2) two terminal branches in the vast majority of specimens at 23 mm.: Temporal Frontal Artery (STA F) and Temporal Parietal Artery (STA P), both are directed towards the epicranial region, and supply the galea, the underlying loose areolar tissue and the bony plane composed of the periosteum and the parietal bone through numerous perforators, the vast majority of which emerge of the distal part of both terminal arteries (mean 51.6). The STA also provides three colateral branches, one of them the Temporal Media Artery (STA M), perforates the fascia and the temporalis muscle reaching the periosteum in the temporal fossa, establishing anastomotic networks with the perforator arteries of the (STA F and P). It is inconstant. The angle of rotation of the superficial temporal vascular axis using the STA F and the Frontal Temporal Vein (FTV) can be up to 120°, allowing coverage of the ipsilateral side of the skull and face without generating vascular folds that could do unviable.
The innervation of auriculotemporal nerve (ATN) in the temporoparietal region coincided with the classic descriptions, its most important relationship being first dorsally with the STV and the STA, and after the division of the latter, with the STA P and the Temporal Parietal Vein (TPV) being substantive for the suggestion of the anatomical bases of its surgical application.
In conclusion, it can be stated that the originality of the proposal lies in the fact that from the location of the territories with the greatest vascularization of the osteofascial plane of the temporoparietal region, the best usable area was identified - in the laterosuperior part of the skull from an initial point of 2 cm. outside the sagittal suture and 2 cm. behind the coronal suture, with a maximum extension, from ventral to caudal ≤ 13 cm., and from medial to lateral ≤ 5 cm., obtaining in total an average maximum surface of 65 cm2 - to make a new osteofascial flap whose vascular axis is given by the superficial temporal vessels, using the numerous distal perforating arterial branches of a terminal branch (STA F or STA P) and the good venous drainage (STV or PTV) and its tributaries. This flap is made up of: superficial temporoparietal fascia, epicranial galea, underlying loose areolar tissue and the bone plane: periosteum, external cortex and diploe of the parietal bone and improves the anatomo-functional and aesthetic results in both the donor and recipient areas when applied in reconstructive surgeries of the face and skull. Another application of the knowledge obtained would be to optimize the already existing regional flaps, which are numerous.
