Caracterización del estrés en padres de neonatos prematuros internados en unidad cuidados intensivos neonatal del hospital Julio C. Perrando
Resumen
La inmadurez del recién nacido prematuro repercute en su estabilidad hemodinámica, requiriendo soportes e intervenciones de salud en Unidades de Cuidados Intensivos (UCIN). La hospitalización del recién nacido prematuro en UCIN es una experiencia impactante e inesperada para los padres, de gran impacto psicológico ante la inminente gravedad de su hijo. La UCIN se transforma en el “Ambiente” del recién nacido y de sus padres. El objetivo del siguiente trabajo fue caracterizar el nivel de estrés de los padres de niños prematuros durante la hospitalización en UCIN.
Material y Métodos: Se realizó un estudio descriptivo, observacional, de corte transversal, en el cual se incluyó a madres y/o padres de recién nacidos prematuros (menores a 37 semanas de edad gestacional), cuyos días de hospitalización fueron posteriores al quinto día de vida y antes de los 30 días desde su nacimiento, internados en UCIN en el Hospital Perrando de la ciudad de Resistencia, período Julio 2019-Enero 2020. Se obtuvo una muestra de 63 padres participantes. Como instrumento de recolección de los datos se utilizó la Escala de Estrés Parenteral en UCIN: Parental Stressor Scale: Neonatal Intensive Care (PSS: NICU) de Miles (1993) adaptada al español. La variables que se analizó fue: Nivel de Estrés de los Padres con respecto a factores estresantes: del “Ambiente Físico de UCIN”, “Aspecto Físico y Comportamiento” del recién nacido prematuro y “Rol Parental”.
Resultados: El Nivel Total de estrés promedio se evidenció como moderadamente estresante con una Media de 2.9 DS: 0.2. El nivel de estrés de los padres en la Subescala de “Aspectos Visuales y Sonoros” tuvo una Media de 2.9 DS: 0.1, en la Subescala de “Aspectos y Comportamientos del bebé la Media fue de 2.6 DS: 0.2 y para la Subescala de “Rol Parental” la Media fue de 3.5 DS: 0.2. El nivel “Extremadamente estresante” estuvo en un 47 % dado por la dimensión de “Rol Parenteral”, seguido en un 43% por la dimensión “Aspectos y Comportamiento del bebé” y en un 10% por el “Aspecto Visual y Sonoro de UCIN”. En el Rol Parental las características más relevantes fueron: “estar separado de mi bebé” (70%), “no poder tomarlo en brazos cuando yo quiero” (63%), “sentirme impotente e incapaz de proteger a mi bebé del dolor” (56%). En Aspectos y Comportamientos del bebé las características de mayor nivel de estrés fueron: “ver agujas y tubos puestos en mi bebé” (38%), “cuando mi bebé parece tener dolor” (37%) y “tener una máquina o respirador que respira por mi bebé” (33%). En Aspectos Visuales y Sonoros la presencia de “ruidos repentinos de alarmas y monitores” fue en un 28.6% extremadamente estresante.
Conclusión. La mayoría de los padres tienen un nivel de estrés total moderado. La dimensión: “Rol Parental” es la más relevante dentro de los niveles extremadamente estresante, debido a que la hospitalización del bebé prematuro en UCIN produce en los padres, alteración en su rol como cuidadores primarios, sintiéndose incapaces de proteger al bebé en esta etapa y dificulta el vínculo madre/padre- bebé. The immaturity of the premature newborn affects their hemodynamic stability, requiring supports and health interventions in Intensive Care Units (NICU). The hospitalization of the premature newborn in the NICU is a shocking and unexpected experience for parents, with great psychological impact given the imminent severity of their child. The NICU becomes the "Environment" of the newborn and its parents.
Material and Methods: A descriptive, observational, cross-sectional study was carried out, which included mothers and/or fathers of premature newborns (less than 37 weeks of gestational age), whose days of hospitalization were after the fifth day of life and before 30 days from birth, admitted to the NICU at the Perrando Hospital in the city of Resistencia, period 2019-2020. A sample of 63 participating parents was obtained. The NICU Parental Stressor Scale: Neonatal Intensive Care (PSS: NICU) by Miles (1993) adapted to Spanish was used as a data collection instrument. The variable that was analyzed was: The level of parental stress regarding of the "Physical Environment of the NICU", "Physical Appearance and Behavior" of the premature newborn and "Parental Role".
Results: The average Total Stress Level was evidenced as moderately stressful with a mean of 3.4 SD: 0.2. The stress level of the parents in the Subscale of "Visual and Sound Aspects" had a Mean of 2.9 SD: 0.1, in the Subscale of "Aspects and Behaviors of the baby the Mean was 2.6 SD: 0.2 and for the Subscale of “Paternal Role” the Mean was 3.5 SD: 0.2. The "Extremely stressful" level was 47% given by the dimension of "Parental Role", followed by 43% by the dimension "Aspects and Behavior of the baby" and 10% by the "Visual and Sound Aspect of the NICU". ” In The Parental Role, the most relevant characteristics were: "being separated from my baby" (70%), "not being able to hold him when I want" (63%), "feeling helpless in being unable to protect my baby from pain" (56%). In Baby Aspects and Behaviors, the characteristics with the highest level of stress were: "seeing needles and tobes put in my baby" (38%), "when my baby seems to be in pain" (37%) and "having a machine or respirator that breathe for my baby” (33%). In Visual and Sound Aspects, the presence of "sudden noises from alarms and monitors" was extremely stressful in 28.6%.
Conclusion: most parents have a moderate total stress level. The dimension: “Parental Role” is the most relevant within the extremely stressfull, because the hospitalization of the premature baby in the NICU causes the parents to alter their role as primary caregivers, feeling incapable of protecting the baby at this stage and hinders the mother/father-baby bond.
