Nuevo método ecocardiográfico para la evaluación de la estenosis mitral reumática
Resumen
Introducción.
La estenosis valvular mitral como secuela de la fiebre reumática sigue siendo un problema de salud en Argentina y especialmente en nuestra región. El diagnóstico se confirma mediante ecocardiograma Doppler, a través de la planimetría directa del área valvular, siendo este método el patrón oro en la actualidad. Sin embargo, debido a factores técnicos y del paciente, la mayoría de las veces se emplea otro método, el tiempo de hemipresión (THP), más simple pero también más expuesto a errores.
El nuevo método propuesto relaciona la velocidad que se registra antes de la válvula y la velocidad a través de la estenosis. Este índice de aceleración de la válvula mitral (IAVM) permitiría graduar la severidad de la valvulopatía.
Por lo tanto, el objetivo de esta investigación es evaluar la utilidad de un nuevo método diagnóstico para estimar la gravedad de la estenosis valvular mitral reumática, compararlo con el tiempo de hemipresión e identificar el mejor punto de corte para diagnosticar estenosis mitral moderada a grave.
Metodología y resultados:
Se incluyeron 145 pacientes quienes fueron derivados al Instituto de Cardiología de Corrientes “Juana F. Cabral” para la evaluación de valvulopatía mitral. Todos debían tener menos de 2 cm2 de área valvular mitral por planimetría. En todos ellos se estimó el área valvular mitral mediante tiempo de hemipresión y se midió el IAVM. Todos los pacientes dieron su consentimiento informado escrito para participar en el estudio.
Se clasificó a los pacientes mediante el área valvular por planimetría en 2 grupos,
A: área valvular mayor a 1,5 cm2 (estenosis mitral leve).
B: área valvular igual o menor a 1,5 cm2 (estenosis mitral moderada-grave).
Se realizó un análisis de curvas ROC (receiver operating characteristic) comparando THP y el IAVM. El área bajo la curva fue 0,75 para THP y 0,83 para IAVM. El punto de corte con la mejor combinación de sensibilidad y especificidad para identificar a pacientes con estenosis mitral moderada a grave fue de 5.
Conclusiones:
El IAVM es útil para identificar pacientes con estenosis mitral reumática moderada a grave, un valor de 5 o más combina la mejor sensibilidad y especificidad. Introduction.
Mitral valve stenosis as a sequel to rheumatic fever continues to be a health problem in Argentina, especially in our region. The diagnosis is confirmed by Doppler echocardiography, through direct planimetry of the valve area, this method is the gold standard today. However, due to technical and patient factors, most often another method is used, the pressure half-time (THP), which is simpler but also more prone to errors.
The newly proposed method relates the velocity that is recorded before the valve and the velocity through the stenosis. This mitral valve acceleration index (IAVM) would allow grading the severity of valve disease.
Therefore, the objective of this research is to evaluate the usefulness of a new diagnostic method to estimate the severity of rheumatic mitral valve stenosis, compare it with the pressure half-time, and identify the best cut-off point to diagnose moderate to severe mitral stenosis.
Methodology and results:
A total of 145 patients were included, who were referred to the "Juana F. Cabral" Institute of Cardiology of Corrientes for evaluation of mitral valve disease. All had to have less than 2 cm2 of mitral valve area by planimetry. In all of them, the mitral valve area was estimated by half-pressure time and the IAVM was measured. All patients gave their written informed consent to participate in the study.
Patients were classified by valve area by planimetry into 2 groups,
A: valve area greater than 1,5 cm2 (mild mitral stenosis).
B: valve area less or equal than 1,5 cm2 (moderate-severe mitral stenosis).
An analysis of ROC (receiver operating characteristic) curves were performed comparing THP and IAVM. The area under the curve was 0.75 for THP and 0.83 for IAVM. The cut-off point with the best combination of sensitivity and specificity to identify patients with moderate to severe mitral stenosis was 5.
Conclusions:
The IAVM is useful to identify patients with moderate to severe rheumatic mitral stenosis, a value of 5 or more combines the best sensitivity and specificity.
