Estudio de la variación de la concentración del potasio en el humor vítreo bovino durante el intervalo post mortem
Resumen
El estudio del intervalo post mortem (IPM) mediante la determinación de la concentración de
potasio en el humor vítreo ha sido y es objeto de investigación en la medicina forense y de otras
disciplinas interesadas en el tema. Las técnicas se desarrollan y mejoran constantemente, y se han
aplicado gradualmente en la ciencia forense.
El humor vítreo tiene ventajas sobre el análisis de sangre, líquido cefalorraquídeo, líquido
sinovial y líquido pericárdico. Se considera que otra de las ventajas por la que los investigadores lo
prefieren como elemento para determinar el intervalo post mortem es debido a su única ubicación
anatómica. Se ha demostrado que el potasio exhibe un aumento después de ocurrida la muertey, hasta la
fecha, todas las investigaciones han arrojado una variedad de relaciones lineales; es decir, diferentes
pendientes estimadas de regresión.
Probablemente, esta carencia se deba en parte a la cantidad variable de casos informados de un
estudio a otro, a los diferentes rangos observados de potasio y de intervalo post mortem y, especialmente,
a que en la mayoría de los trabajos realizados tuvieron la dificultad de no contar con la hora exacta en que
ocurrió la muerte, y a la que debieron arribar como conclusión a través de estimaciones en el análisis de
otros indicadores o de registros realizados por otras personas ajenas a la investigación.
A los fines de contar con exactitud con el momento en que ocurrió la muerte, se han estudiado
60 humores vítreos recolectados de ojos enucleados de ganado bovino al momento del sacrificio. Las
muestras vítreas se analizaron para determinar la concentración de potasio al momento de la muerte y a
las dos, seis y diez horas posteriores.
De cada animal, a los tres minutos de la muerte por electrocución, se extrajo la primera muestra
de humor vítreo y luego se continuó extrayendo muestras hasta completar las diez horas. Las puncionesaspiraciones
fueron totalmente indoloras ya que los animales fueron sacrificados previamente. Los
volúmenes de muestras fueron entre 95μL y 65μL por punción.
Las concentraciones de potasio aumentaron con el tiempo. Las concentraciones medias de
potasio medidas en los dos ojos de todos los bovinos en el presente estudio variaron de 6,71 a 7,83
mEq/L con los valores del intervalo post mortem de 0 a 10 horas. Se encontró una correlación positiva
entre la concentración de potasio y la hora de medición.
En el caso de los ojos derechos la magnitud de la correlación positiva fue de 0,478 y
estadísticamente significativa (p0,05). Para los ojos izquierdos, la magnitud de la correlación positiva
fue más alta, con un valor de 0,655, y también estadísticamente significativa (p0,05).
En virtud de la correlación positiva y significativamente obtenida, se construyó un modelo de
regresión lineal para brindar una función predictora a través de una nueva fórmula de cálculo, la que fue
conseguida con mejor ajuste, según los datos obtenidos, de los ojos izquierdos.
Sin embargo, la determinación de la concentración de potasio en el humor vítreopost
mortemestá limitada debido a la disparidad en los resultados de los distintos trabajos publicados y la falta
de uniformidad en las ecuaciones propuestas.Por consiguiente, podemos deducir que se necesitan más
investigaciones para poder superar las dificultades. The study of post mortem interval (IPM) by determining the concentration of potassium in the
vitreous has been and is being investigated in forensic medicine and other disciplines interested in the
subject. The techniques are developed and constantly improved, and have been applied gradually in
forensic science.
Vitreous humor has advantages over the analysis of blood, cerebrospinal fluid, synovial fluid
and pericardial fluid. It is considered that another of the advantages for which researchers prefer it as an
element to determine the post mortem interval is due to its unique anatomical location. It has been shown
that potassium exhibits an increase after death has occurred and, to date, all investigations have yielded a
variety of linear relationships; that is, different estimated slopes of regression.
This inconsistency is probably due in part to the variable number of cases reported from one
study to another, to the different observed ranges of potassium and post-mortem interval and, especially,
to the fact that in most of the studies carried out they had the difficulty of not to count on the exact hour in
which the death occurred, and to which they had to arrive as a conclusion through estimations in the
analysis of other indicators or of records made by other people outside the investigation.
In order to have accurate account of when the death occurred with the moment in which death
occurred, 60 vitreous humors collected from enucleated eyes of cattle at the time of slaughter were
studied. The vitreous samples were analyzed to determine the concentration of potassium at the time of
death and at two, six and ten hours later.
From each animal, three minutes after death by electrocution, the first sample of vitreous was
extracted and then samples continued to be taken until the end of ten hours. The aspirations were
completely painless since the animals were previously sacrificed. Sample volumes were between 95μL
and 65μL per puncture.
Potassium concentrations increased over time. The mean potassium concentrations measured in
the two eyes of all cattle in the present study ranged from 6.71 to 7.83 mEq / L with post-mortem values
from 0 to 10 hours. A positive correlation was found between the potassium concentration and the time of
measurement.
In the case of right eyes, the magnitude of the positive correlation was 0.478 and statistically
significant (p 0.05). For the left eyes, the magnitude of the positive correlation was higher, with a value
of 0.655, and also statistically significant (p 0.05).
Given the positive and significant correlation obtained, a linear regression model was
constructed to provide a predictive function through a new calculation formula, which was achieved with
a better fit, according to the data obtained, of the left eyes.
However, the determination of the potassium concentration in the post mortem vitreous humor
is limited due to the disparity in the results of the different published works and the lack of uniformity in
the proposed equations. Therefore, we can deduce that more research is needed to overcome the
difficulties.
